Social Prescribing

Social Prescribing is a new service helping people access a wide variety of local community services in the borough. The Runnymede Wellbeing Adviser will work with residents to help find suitable local services with the aim of improving their wellbeing. This service is available after a GP or health practitioner refers the individual for additional non-medical support and advice.

Who is this service suitable for?

Adults over 18 years of age.

Services you may be signposted to

  • Mental or emotional support (including Mind Matters)
  • Memory loss or dementia services and groups
  • Staying independent at home (alarms, meals, handyman etc.)
  • Opportunities for social interaction (Social Centres)
  • Getting out and about (transport)
  • Practical help (cooking, cleaning, shopping)
  • Local interest groups (bingo, craft, fishing etc.)
  • Leisure activities (swimming, walking etc.)
  • Family support
  • Benefits/finance/budgeting
  • Housing
  • Support around a disability
  • Exercise programmes

Wellbeing Appointments

Anyone referred to the Wellbeing Adviser will be contacted to arrange a face to face appointment. Where possible, appointments will take place in a confidential setting at one of our three Social Centres in the borough. Home appointments will be offered where this is required.

You will be contacted six weeks after your appointment for a further discussion and any services or groups accessed will be recorded as well as completing a wellbeing survey.

Sharing of information

With consent, individuals contact details will be shared with other agencies for the purpose of supplying specific services requested only.

Looking after your data

In order to provide the Wellbeing Prescribing Service we will collect and process personal data and special category data, such as health and disability data relating to you. We will need to process your personal data as part of the monitoring of performance in providing the Wellbeing Prescribing service. We also need to process your data to enable referrals or requests for services or support from other organisations to be provided. This will be agreed with you in advance and is based on your consent.
