Patient situation
You will have the following options available to select:
In hospital - planned discharge - borough services required

This should be used for all patients who would benefit from borough council services on discharge. This should be the standard pathway for any patient admitted for longer than 48 hours.
In hospital - prevention of admission/urgent discharge (within 48 hours)

This option should be selected when one of the following situations occurs

Where a patient's stay in hospital is less than 48 hours

Where there is an unexpected change to a patient's discharge time

Where a patient is discharged out of hours e.g. evenings, weekends or Bank Holidays and a pro-active referral was not made

At home - community-based referral

This option should be selected for any referral where a patient is not currently in hospital
Patient Information
When inputting the patient's address, the appropriate borough should be selected. This will determine which borough referral form will be sent to on submission.
If a patient has an existing key safe, the information should be inputted on this page.
Discharge details and medical information
If the patient's discharge date and time is known this should be completed as accurately as possible to ensure borough staff are able to provide the relevant services in a timely manner.
If the discharge date is not known a rough estimation should be given e.g. two weeks Medical Need - the most relevant category should be selected. This may not reflect the reason for their hospital stay which can be detailed underneath.
Where possible do not use medical abbreviations as staff receiving these referrals will not be medically trained and may not understand the terms used. Please provide sufficient information to cover issues/disabilities that may need to be relayed to the ambulance service if the patient falls unwell.
Next of kin information
Up to two emergency contacts can be entered. One of these contacts should preferably be living nearby to the patient to assist with access to the property should there be an emergency or concern for welfare.
If there are no next of kin, the patient can still be referred for Homesafe Plus, however if there is not an existing key safe, a temporary key safe must be selected when this option is given.
Community Alarm
If a community alarm is needed you will be guided through the form to complete details for a member of staff to arrange installation of an alarm or to give the patient an Emergency Homesafe pack.
Temporary key safe
This page may appear in certain scenarios. If needed, you will be guided to provide the patient with one of the special temporary key safe only packs.
Meals at Home
If Meals at Home are required, you will be guided through the form to add any dietary and allergy information.
Staff will need to know how access to the patient's property will be made. If there is no one able to answer the door you will be prompted to indicate whether access should be made through one of the emergency contacts given, an existing key safe or a temporary key safe.
Handyperson and Home Improvement Agency
Use this page to select any equipment or adaptations that the patient may require following their hospital stay. An assessment will be carried out by the borough council to determine suitability.
Referrer details
If an email address is inputted, a copy of the referral submission will be sent to it.
