- full details about the complaint; when, where, and what happened
- which part of the Councillor’s code of conduct you feel has been breached (if known)
- your preferred contact details
- you need to tell us if you do not wish your name to be disclosed to the Councillor
Note
You get 10 minutes to complete each page of the form so you might want to prepare your complaint in Word and then copy and paste it into the form. If you have a ‘my account’ with us this time limit does not apply.
